§ 64.5001   Reporting and certification requirements.

   On a quarterly basis, every prepaid calling card provider must submit
   to the Commission a certification with respect to the prior quarter,
   signed by an officer of the company under penalty of perjury, stating
   that it is making the required Universal Service Fund contribution.
   This provision shall not apply to any prepaid calling card provider
   that has timely filed required annual and quarterly Telecommunications
   Reporting Worksheets, FCC Forms 499-A and 499-Q, during the preceding
   two-year period.
